PLAN DETAILS
- 12-month membership fee is due in full upon joining the plan
- Membership is effective the day the membership fee is received
- It is the responsibility of the membership owner to maximize their benefits by arranging all appropriate appointments within the 12-month membership period
- If benefits are not used, the member will NOT be entitled to a refund
- Renewal payment is due on the anniversary date of your membership each year
Additional Details
- Our In-House Savings Plan is NOT dental insurance
- The membership can ONLY be used at Organ Mountain Dental with Dr. Jake B. Wolf
- This membership cannot be combined with any other dental insurance coverage or adjustments
